Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It is a platform that collects and shares clean trial data and ratings using PRECIS-2, allowing for multiple and diverse meta-epidemiological research studies to evaluate the effect of treatment on trials with different levels of pragmatism as well as other design features.
Background
Pragmatic trials are increasingly acknowledged as providing evidence from the real world for clinical decision-making. The term "pragmatic" however, is not used in a consistent manner and its definition and assessment require further clarification. Pragmatic trials are designed to inform clinical practices and policy decisions, not to confirm a physiological hypothesis or clinical hypothesis. A pragmatic study should try to be as similar to real-world clinical practice as is possible, including the recruitment of participants, setting up and design as well as the implementation of the intervention, and the determination and analysis of the outcomes, and primary analyses. This is a major distinction between explanatory trials as described by Schwartz and Lellouch1, which are designed to test the hypothesis in a more thorough manner.
Truly pragmatic trials should not conceal participants or clinicians. This can result in a bias in the estimates of the effects of treatment. Pragmatic trials should also seek to enroll patients from a wide range of health care settings, so that their results can be compared to the real world.
Additionally studies that are pragmatic should focus on outcomes that are crucial to patients, such as quality of life or functional recovery. This is particularly relevant in trials that involve the use of invasive procedures or potential for serious adverse events. In addition to these features pragmatic trials should reduce the trial's procedures and data collection requirements to reduce costs. Furthermore pragmatic trials should try to make their results as applicable to real-world clinical practice as possible by making sure that their primary method of analysis follows the intention-to treat approach (as described in CONSORT extensions for pragmatic trials).
Many RCTs that do not meet the criteria for pragmatism but have features that are contrary to pragmatism have been published in journals of varying types and incorrectly labeled as pragmatic. This can lead to misleading claims of pragmatism and the term's use should be standardized. The creation of a PRECIS-2 tool that offers an objective, standardized evaluation of pragmatic aspects is a good start.
Methods
In a pragmatic study it is the intention to inform clinical or policy decisions by showing how an intervention could be integrated into routine care in real-world settings. This is distinct from explanation trials that test hypotheses about the causal-effect relationship in idealized situations. Therefore, pragmatic trials might have lower internal validity than explanatory trials and may be more susceptible to bias in their design, conduct, and analysis. Despite their limitations, pragmatic research can provide valuable information to make decisions in the context of healthcare.
The PRECIS-2 tool scores an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatist). In this study, the recruit-ment, organization, flexibility in delivery and follow-up domains received high scores, but the primary outcome and the procedure for missing data fell below the pragmatic limit. This suggests that a trial can be designed with well-thought-out practical features, yet not compromising its quality.
It is, however, difficult to assess how practical a particular trial is, since the pragmatism score is not a binary attribute; some aspects of a trial can be more pragmatic than others. Additionally, logistical or protocol changes during a trial can change its score in pragmatism. Additionally 36% of the 89 pragmatic trials identified by Koppenaal and co. were placebo-controlled or conducted prior to licensing, and the majority were single-center. Furthermore, pragmatic studies can present challenges in the collection and interpretation of safety data. This is due to the fact that adverse events are usually self-reported and are susceptible to errors, delays or coding differences. It is therefore important to improve the quality of outcome assessment in these trials, and ideally by using national registries instead of relying on participants to report adverse events in the trial's database.
Results
Although the definition of pragmatism does not require that all trials are 100% pragmatic, there are advantages to including pragmatic components in clinical trials. These include:
By including routine patients, the results of trials are more easily translated into clinical practice. However, pragmatic studies can also have disadvantages. The right type of heterogeneity, like could allow a study to generalise its findings to many different patients or settings. However the wrong kind of heterogeneity can reduce the sensitivity of an assay and thus lessen the power of a trial to detect minor treatment effects.
A variety of studies have attempted to categorize pragmatic trials with a variety of definitions and scoring systems. Schwartz and Lellouch1 created a framework to distinguish between research studies that prove the clinical or physiological hypothesis, and pragmatic trials that help in the choice of appropriate therapies in clinical practice. Their framework included nine domains, each scoring on a scale ranging from 1 to 5 with 1 indicating more lucid and 5 indicating more pragmatic. The original PRECIS tool3 had similar domains and a scale of 1 to 5. Koppenaal and colleagues10 developed an adaptation to this assessment dubbed the Pragmascope which was more user-friendly to use in systematic reviews. They discovered that pragmatic systematic reviews had higher average scores across all domains, with lower scores in the primary analysis domain.

This distinction in the primary analysis domains can be explained by the way that most pragmatic trials approach data. Some explanatory trials, however don't. The overall score was lower for systematic reviews that were pragmatic when the domains of organisation, flexible delivery, and follow-up were merged.
It is important to note that a pragmatic trial doesn't necessarily mean a poor quality trial, and in fact there is an increasing number of clinical trials (as defined by MEDLINE search, but it is neither specific or sensitive) that use the term "pragmatic" in their title or abstract. The use of these terms in titles and abstracts could indicate a greater understanding of the importance of pragmatism but it isn't clear if this is reflected in the content of the articles.
Conclusions
In recent times, pragmatic trials are becoming more popular in research as the value of real world evidence is increasingly recognized. They are randomized trials that compare real world treatment options with clinical trials in development. They are conducted with populations of patients closer to those treated in regular medical care. This method is able to overcome the limitations of observational research, like the biases that come with the reliance on volunteers, and the limited availability and the coding differences in national registry.
Other benefits of pragmatic trials include the ability to utilize existing data sources, and a higher chance of detecting meaningful changes than traditional trials. However, these trials could be prone to limitations that compromise their validity and generalizability. The participation rates in certain trials could be lower than anticipated due to the healthy-volunteering effect, financial incentives or competition from other research studies. A lot of pragmatic trials are restricted by the necessity to recruit participants quickly. In addition some pragmatic trials don't have controls to ensure that the observed differences are not due to biases in the conduct of trials.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs self-labeled as pragmatic and were published up to 2022. The PRECIS-2 tool was employed to assess pragmatism. It covers areas like eligibility criteria, recruitment flexibility and adherence to intervention and follow-up. They found that 14 of these trials scored highly or pragmatic practical (i.e. scoring 5 or higher) in any one or more of these domains and that the majority of these were single-center.
Studies with high pragmatism scores tend to have more criteria for eligibility than conventional RCTs. They also contain populations from many different hospitals.
